Перигравідарний менеджмент ревматичних захворювань

Менеджмент запальних ревматичних захворювань при плануванні і під час вагітності та грудного вигодовування за останні кілька років зазнав значних змін. Сучасна терапія, включаючи біологічні та таргетні синтетичні хворобо-модифікувальні препарати, дозволили істотно поліпшити контроль ревматичних захворювань, що привело до збільшення кількості жінок з тяжкими захворюваннями, які планують вагітність і вагітніють. Під час консультації таких пацієнток необхідно обговорення можливого часу зачаття (щодо активності захворювання), впливу захворювання на вагітність і вагітності на нього, а також потенційної необхідності зміни режиму вживання лікарських препаратів під час вагітності і годування груддю. У цьому огляді узагальнено інформацію про вплив вагітності на різні ревматичні захворювання і, навпаки, про зміни терапії і моніторинг пацієнток з ревматичними захворюваннями до, під час і після вагітності.
Жінки із запальними ревматичними захворюваннями потребують консультацій щодо медикаментозної терапії перед плануванням вагітності, під час вагітності і годування груддю та повинні усвідомлювати ризик післяпологових ускладнень. Безпечними хворобо-модифікувальними препаратами, вживання яких можливе під час вагітності, є гідроксихлорохін, сульфосалазин, азатіоприн і циклоспорин. Під час вагітності також можливе вживання глюкокортикоїдів і нестероїдних протизапальних препаратів до 32-го тижня гестації. Більшість інгібіторів фактора некрозу пухлини (анти-ФНП) також є безпечними у період вагітності. Під час вагітності необхідний чіткий моніторинг активності захворювання, контроль рівня аутоантитіл, насамперед анти-SSA / Ro і анти-SSB / La і антифосфоліпідних антитіл, оцінювання ступеня дисфункції органів, особливо ураження нирок.
Представлені сучасні підходи до оптимізації менеджменту запального ревматичного захворювання під час вагітності. Для пацієнтів із запальними ревматичними захворюваннями успішний результат вагітності оптимізується шляхом створення індивідуального плану пригнічення й контролю активності хвороби з використанням цільового підходу.
