Монозиготні багатоплідні вагітності після допоміжних репродуктивних технологій (огляд літератури)

Частота виникнення монозиготних багатоплідних вагітностей (МБВ) після допоміжних репродуктивних технологій (ДРТ) є значущо вищою, ніж у загальній популяції.
Мета цього систематичного огляду — вивчити вплив віку, контрольованої стимуляції яєчників, маніпуляцій на ZP; кріоконсервування та пролонгованого культивування ембріонів in vitro до стадії бластоцисти на частоту виникнення МБВ.
Оригінальні дослідження та огляди отримано за допомогою пошуку баз даних PubMed, Embase та Cochrane. У результаті пошуку літератури проаналізовано 91 статтю, у тому числі 42 оригінальні дослідження. Через незначну кількість даних неможливо відокремити фактор впливу на частоту виникнення МБВ. Проте порівняння наведених у літературі даних дає змогу зробити висновок про сукупний вплив технологій програм ДРТ на збільшення МБВ. Серед них: стимуляція овуляції; втручання на ZP; затвердіння блискучої оболонки; кріоконсервування передімплантаційних ембріонів на стадії бластоцисти з її попереднім колапсуванням; субоптимальні умови культивування, перенесення в порожнину матки ембріону на стадії бластоцисти. При МБВ значущо частіше виникають акушерські та перинатальні ускладнення, що необхідно враховувати під час вибору тактики ведення таких пацієнтів та потребує від спеціалістів високого професіоналізму та уваги.
