Ефективність діагностики і лікування преінвазивного та мікроінвазивного сквамозоклітинного раку шийки матки

Мета дослідження: аналіз ефективності діагностики та лікування хворих на ранній рак шийки матки.
Матеріали та методи. Проаналізовано дані 107 жінок, хворих на СIN ІІІ, сancer in situ, які знаходились на лікуванні у Київському обласному онкологічному диспансері та Національному інституті раку МОЗ України з 2010 до 2015 року, віком від 20 до 52 років, – історії хвороби, амбулаторні картки, гістологічні препарати.
Результати. Відсоток діагностованих рецидивів СIN ІІІ, сancer in situ становив 4,57%, неінвазивних та інвазивних форм – 0,94%.
Заключення. При встановленні діагнозу СIN ІІ і СIN ІІІ рекомендовано проводити лікування шляхом діатермоконізації та динамічної диспансеризації, застосовуючи комплексну програму (кольпоскопія, цитологія, онкомаркер SСС, виявлення інфікування HPV 16,18).
За наявності у краї резекції елементів пухлини після розширеної діатермоконізації, при формах сancer in situ, множинних експансивних вогнищах у залозах шийки матки, реалізованій репродуктивній функції, супутніх захворюваннях (вузлова лейоміома тіла матки і т.д.) слід рекомендувати гістероскопічну радикальну трахелектомію.
Перспективним є прийняття «принципу продовження спостереження» та скринінгове виявлення інфікування HPV 16,18 при СIN І, СIN ІІ.
