Профілактика великих акушерських синдромів у вагітних з резистентністю до біодоступного прогестерону

У статті представлені сучасні літературні дані про роль дефективної глибокої плацентації у розвитку великих акушерських синдромів, біодоступного прогестерону та його рецепторів у формуванні плаценти і розвитку вагітності. Описані механізми розвитку резистентності до біодоступного прогестерону, наведена схема патогенезу недостатності впливу прогестерону під час вагітності як однієї з основних причин розвитку великих акушерських синдромів.
Висвітлені сучасні дані про розвиток резистентності до біодоступного прогестерону у підлітковому віці, при ендометріоїдній хворобі, синдромі полікістозних яєчників та ідіопатичній рецидивній втраті вагітності. Розглянуто терапевтичні можливості профілактики великих акушерських синдромів при резистентності до біодоступного прогестерону. Наведено дані досліджень про застосування високоселективного прогестагену з підвищеним афінітетом до прогестеронових рецепторів дидрогестерону для профілактики великих акушерських синдромів у вагітних з резистентністю до біодоступного прогестерону.
