The high frequency of cesarean section in women with a history of infertility after the use of assisted reproductive technologies (ART) requires the introduction of programmed management of pregnancy and childbirth with adequate prevention of placental insufficiency (PІ), which will significantly improve the results of childbirth from both obstetric and perinatal perspectives.
Aim – to study the clinical and statistical features of childbirth in pregnant women with various forms of infertility in history in order to develop indications for programmed management of childbirth in them.
Materials and methods. 524 cases of childbirth in pregnant women after ART with various forms of female infertility in history: Group I – 384 pregnant women with an endocrine form, Group II – 791 pregnant women with a mixed form, Group III – 49 pregnant women with a tubal factor. The features of somatic, gynecological and reproductive history, the frequency of pregnancy complications, and methods of reproduction were studied.
Results. The endocrine form occurs in 73.3%; mixed in 17.4%, tubal factor in 9.3% of observations. Secondary infertility occurred in 32.6%, in 43.8% women artificial abortions, in every 6 (15.8%) cases spontaneous miscarriages. Threat of premature birth – 41.2% cases (in Group I – 44.3%, in Group III – 24.5% cases); level of preeclampsia 11.8% cases, with a predominance in Group II – 16.5% cases and in Group III – 16.6%; frequency of PI was 26.1%.
Conclusions. In pregnant women with a history of infertility, the frequency of delivery by cesarean section is 37.9%, with the rate of planned cesarean section being 37.7% and urgent cesarean section being 62.3%.
The study was conducted in accordance with the principles of the Declaration of Helsinki. Informed consent from women was obtained for the study.
The authors declare that there is no conflict of interest.
