War and pregnancy: biological mechanisms, maternal stress and perinatal outcomes

Armed conflicts represent a major factor adversely affecting women’s reproductive health. Pregnancy under such conditions is associated with exposure to multiple adverse factors, including chronic psychological stress, nutritional disturbances, infectious risks, and limited access to healthcare.
Aim – to summarize current scientific evidence on the impact of war-related stress on maternal physiology, pregnancy course, fetal condition, and neonatal health.
An analytical review of current scientific publications and epidemiological studies on pregnancy outcomes in conflict settings was conducted. Chronic stress is associated with neuroendocrine dysregulation, systemic inflammation, and placental dysfunction, which increase the risk of obstetric and perinatal complications. Mental health disorders, including anxiety, depression, and post-traumatic stress disorder, significantly affect pregnancy outcomes. Additional negative factors include disruption of healthcare systems and limited access to antenatal care.
Conclusions. Armed conflicts have a multidimensional impact on maternal and neonatal health. Understanding the underlying biological and social mechanisms is essential for optimizing care and preventing adverse perinatal outcomes in crisis settings.
