Uterine artery doppler as a component of a perinatal monitoring pregnant women with high risk

Goal is on the basis of comparative study of ultrasound and doppler indicators in pregnant women with physiological and complicated course of gestation to distinguish early prognostic markers of physiological disorders of pregnancy.

Patients and methods. The study was conducted in three clinical groups: I group — patients with physiological pregnancy (n=100). Group II — patients with pregnancy complicated by placental dysfunction with signs of delayed fetal growth (n=44); group III — patients with a pregnancy that stopped in development (n=58).

Results. Found that at 11–12 weeks of gestation in the group I indicators, systolic-diastolic relationship in the right and left uterine arteries in these patients was 1.9±0.3 mm and 2.1±0,35, respectively. In pregnant women with adverse outcomes of pregnancy, systolic-diastolic ratio in right and left uterine arteries in group II was 3.0±0.5 and 2.9±0,45. It was found that 72 (72%) patients with uncomplicated at 11–12 weeks of pregnancy in intervillous space blood flow was not registered. In 28 (28%) patients — found continuous venouslike blood flow. Arteriolike pulsating type of flow it is not revealed none of the group II pregnant. In group II in 32 (72.7%) of the pregnant woman were recorded, two types of curve in intervillous space in pulsed and continuous arteriolike venouslike. Only in 12 (27.3%) patients of this group the blood flow in intervillous space was not determined. In intervillous space the overwhelming majority, 46 (79.3%) patients of group III, were recorded arteriopathy venouslike and type of blood flow.

Conclusion. The increased peripheral vascular resistance in both uterine arteries (systolic-diastolic ratio more than 2.5), which is accompanied by the appearance arteriovenous type of blood flow in venouslike space are early markers of formation of obstetric pathology.
