Using reflux-screening questionnaire for screening diagnosis of pathological gastroesophageal reflux in children

Purpose. To evaluate the diagnostic value of reflux-screening questionnaire for screening children which need for additional clinical and instrumental studies to confirm the presence of pathologic gastroesophageal reflux, or gastroesophageal reflux disease.

 

Material and methods. We observed 172 children aged from 3 months to 3 years who were on outpatient and inpatient treatment in a children's hospital number 3 and City Clinical Hospital №9, Odessa during the period from June 2012 to February 2014, coming in at the examination and treatment of various somatic diseases. All children of the studied group were evaluated using state-reflux questionnaire and only patients with more than 7 points wallet to the evaluation recommended further examination of the digestive system (general clinical and biochemical studies, molecular genetic testing to identify gene mutations collagen A3, endoscopic examination of the esophagus and gastric pH monitoring of the esophagus). Efficiency and informative method were evaluated using ROC-analysis, identify indicators of sensitivity (Se, sensitivity) and specificity (Sp, specificity), with 95% confidence intervals. As the «cutoff» for calculating the AUC (area under the curve) was determined reflux score equal profiles 7.

 

Results. The analysis of the assessment results using reflux questionnaire following data were obtained: of 172 children surveyed, 24 (13.9%, 0.29 — 1.66; 95% CI) was observed result of more than 9 points, 37 (21.5% 0.61 — 2.62 95% CI) — 7 points, in 111 (64.5%, 0.9 — 3.188, 95% CI) — 6 points or less.

 

Conclusions. Reflux questionnaire is highly sensitive and specific method for diagnostic screening pathological gastroesophageal reflux in children. After questioning by a reflux profiles it is possible to differentiate the children into groups for further diagnosis because of its invasiveness.
