Purpose — to study the peculiarities of the course of pregnancy in women with infertility of the endocrine and inflammatory genes in history, depending on the method of fertilization.
Patients and methods. The study of the course of pregnancy was conducted in three groups (six subgroups): And the group — pregnant women who had an endocrine infertility history: Ia — 50 patients after the IVF, Ib — 50 patients who conceived independently after conservative and surgical treatment of infertility, but without IVF; The second group — pregnant women who had a history of infertility of inflammatory genesis: IIa — 100 patients after the IVF, ІІb — 100 patients who became pregnant independently after conservative and surgical treatment of infertility, but without IVF; III group — pregnant women who had a history of combined infertility, inflammatory genesis with endocrine, ІІІa — 30 patients after IVF, ІІIb IVF — 30 patients who conceived independently after conservative and surgical treatment of infertility, but without IVF.
Results. Up to 70–80% of patients in all groups most often at the age of 31–40 years applied for conservative and surgical treatment of infertility, as well as to IVF. Analyzing the gynecological history, in pregnant II and III groups there were 60–90% of chronic salpingophoritis, chronic metritis, tubal pregnancies. Also in women of ІІ and ІІІ groups, 15–33.3% of cases were leiomyoma or endometriosis before pregnancy. In pregnant I and III groups, 16.8–23.3% had hypoplasia of the uterus. Pregnant I and III groups had more metabolic complications: gestational diabetes — 32% and obesity — 70%. The highest frequency of threatening early and late miscarriages and premature births was in the I and III groups. In the first trimester, this figure was over 90%. Each fourth pregnancy in group I and every third in group III ended in preterm labor, regardless of the fertilization method. Complications of inflammatory nature in pregnant women were more frequent in ІІ and ІІІ groups.
Conclusions. Thus, the course of pregnancy in women with infertility of different genesis in history is accompanied by a high frequency of obstetric complications, namely: threatening early and late miscarriage, premature delivery, premature outflow of amniotic fluid, hypertension disorders, metabolic disorders not affected by the fertilization method, and which depend on the infertility factor.
