Metabolic syndrome: contemporary views and danger in pregnancy

In this article we present modern views on metabolic syndrome (MS) and its negative influence during pregnancy. МS, according to the World Health Organization experts, is a «pandemic of the XXI century» due to its prevalence among the population, high risk of cardiovascular disease development and premature mortality. In routine clinical practice, the diagnosis of MS is established in the presence of at least three risk factors: abdominal obesity; elevated triglycerides index; low levels of high-density lipoprotein cholesterol; high blood pressure; elevated fasting glucose level. The manifestations of MS include predisposition to thrombus formation, liver steatosis/steatohepatitis, microalbuminuria / nephropathy, encephalopathy, sensorineural hearing loss, gallstone disease, polycystic ovaries, hyperuricemia, obstructive sleep apnea syndrome, osteoporosis, periodontal disease, impotence / sexual dysfunction etc. Influence of negative factors in childhood and adolescence (malnutrition, low physical activity, smoking) lead to the development of MS. MS during pregnancy causes metabolic disorders in the hemostatic system. During pregnancy the risk of thromboembolic complications increases physiologically (according to some data, 4–5 times) as compared to the state before pregnancy. MS is associated with hypercoagulation due to increased plasma activity levels of hemostasis, reduced fibrinolysis, emergence of endothelial dysfunction, and increased platelet activity. These disorders in the system of coagulation and fibrinolysis can be additional factors to worsen the course of pregnancy and increase the risk of thrombotic complications in patients with MS.
