Implementation of the pilot project (pooling of financial resources, differentiation of financial resources by types of medical care): preferences and risks

Purpose. Implementation evaluation the introduction of new financial and economic mechanisms in the pilot regions, in particular the creation of a more powerful financial «pools», the differentiation of expenses of medical care and the possibility of dissemination of experience on the healthcare system in Ukraine.

Materials and methods. Analyzed the provisions of relevant laws and regulations, monitoring data of indicators for the assessment of the health care reform in pilot regions . Used analytical, comparative methods.

Results. The research found that the combination of financial resources at the level of the oblast budget has allowed to provide the principle exterritoriality, fairness specialized medical care, to form a more powerful financial pools to provide guarantees for the population. In particular, in Dnipropetrovsk oblast are provided 100% funding for emergency care, increased by 46.3% financing provision of preferential categories of the population free of drugs on prescription, shows expenditure of drugs and nutrition in the unified standards and increase their more than 2 times. Funding reduced economic classification of expenditures at the level of managers of the lowest level allowed to Finance health facilities that provide mine action activities in the pilot regions for two codes of economic classification of expenditures. It has allowed to simplify and reduce the workflow in health care facilities; to accelerate the payment of payment orders to the agencies of the State Treasury service of Ukraine, to increase the efficiency of introducing changes in the painting of the appropriations for the stewards of the lowest level, to simplify the procedure of budget transfers in part changes plans for the use of budget funds; to determine the priority of payments.

Conclusions. Concentration at the level of oblast budget financial, human and material resources allows you to legally provide the routing of the patient according to the need in the direction of medical care, the implementation of common approaches to financing institutions, the selection of priority measures, reduction of financial risks for patients. Using funding for two codes of economic classification of expenditures in health care settings regardless of the type of medical care is a positive innovation and can be distributed to health facilities irrespective of a kind of medical care on the territory of Ukraine.
