Comparative evaluation of the medical and surgical management of early reproductive losses

The objective: to carry out a comparative assessment effectiveness of medical and surgical interruption of missed abortion in the early stages of gestation.
Materials and methods. A total of 100 pregnant women with missed abortion aborted miscarriage in terms of pregnancy 5-9 weeks were examined. The control group included 30 women with singleton pregnancy, which normally develops in accordance with the terms of gestation.
For the purpose of interrupting the missed abortion, a combination of preparations of mifepristone 600 mg and misoprostol 400-800 mg was administered vaginally or sublingually with a 36-48-hour interval.
Results. It was established that missed abortion were combined with a significant violation of the microecology of the genital tract, the frequency of associative forms of bacterial infection of genitalium was seeds in 73% of cases, on the background of an increase in the frequency of diagnosis of G. vaginalis (33%), U. urealaticum (27%), M. hominis (21%) and Ch. trahomatis (18%). The effectiveness of medical abortion was 98%, which corresponded to the effectiveness of the surgical abort method, with the frequency of complications being three times less in the case of medical abortion, which indicated the safety and priority of the use of this method in order to preserve the reproductive health of women with early pregnancy loss.
Conclusion. The medical method of management of early reproductive losses is a safe alternative to surgical abortion. The use of the medical method in 98% of cases allowed avoiding the more traumatic instrumental products of conception, which is a priority in order to preserve the reproductive function of women.
