Characteristics of the hemostatic system in patients with abnormal uterine bleeding at puberty

Aim of the study — to investigate the coagulation and anticoagulation systems of hemostasis in adolescent girls with abnormal uterine bleeding (AUB).

 

Patients and methods. A clinical and hemostasiologic study was carried out in 184 patients aged 11–18 years with AUB. We determined the parameters of coagulation, anticoagulation and fibrinolytic systems of hemostasis: platelet count, fibrinogen and fibrinogen B levels, plasma recalcification time, prothrom bin index, plasma tolerance to heparin as well as blood fibrinolytic activity.

 

Results. Some deviations in the blood coagulation system (90.2% in gr. I, 90.0% in gr. II, and 94.4% in gr. III) turned out to be typical for patients with AUB. They are differently directed from hypercoagulation right up to anticoagulation phenomena. Manifestations of hypercoagulation have been observed in gr. I patients (58.1%), significantly more frequently they have been registered in gr. II patients (66.7%), and significantly less frequent they have been found in gr. III adolescents — 50.7% (p<0.05). Manifestations of anticoagulation have been revealed more often in gr. III patients — 43.7% (p1.2<0.01), less frequent they have been registered in patients in gr. II — 23.3% (p1.2<0.001). In adolescents with AUB (gr. I) manifestations of the kind have been observed in 32.0% of cases. No deviations have been found in the system of hemostasis in 9.8% of patients in gr. I, 10.0% in gr. II patients, and much less (5.6%, p1,2<0.01) in gr. III girls.

 

Conclusion. Identification of disorders in the hemostatic system will make it possible to use in good time differentiated and efficient medications, designed to hold up uterine bleeding, and will contribute to its faster termination.
