Purpouse — study the features of the gastric mucosa protective barrier in children with chronic gastroduodenitis depending on the duration of the disease.
Patients and methods. Under our supervision there were 76 children aged 8 to 16 years. For verifaing the diagnosis, all children were performed endoscopy upper gastrointestinal tract, histological and immunohistochemical study of gastric mucosa.
Results. In histological examination of biopsies of the gastric mucosa in most patients with newly diagnosed CGD irregularities were detected gastric cytoprotective functions with the changes in local homeostasis of the gastric mucosa. During the recurrent chronic gastroduodenitis, it’s often noted the violations of gastric barrier function with the stromal-epithelial adjustment of the gastric mucosa. In immunohistochemical study of gastric biopsies in the majority of children recently diagnosed with chronic gastroduodenitis expression of receptor VEGFR-1 and collagen type IV was moderate, and during the recurrent disease course, the level of positively stained cells to receptor VEGFR-1 and collagen type IV was weak. In a comparative evaluation of the degree of color neutral and acid mucopolysaccharides in the gastric mucosa biopsies according to the duration of the disease, we`ve noticed that most of the epithelial barrier violations of the gastric mucosa was found in patients with recurrent course chronic gastroduodenitis. Epithelial level of the gastric mucosa protection was not dependent on the length of chronic gastroduodenitis, because it was noticed moderate level of expression of PGE2 in most patients.
Conclusion. According to the study, we have identified features of changes in protective mucosal barrier of the gastric mucosa that affected the formation and progress of chronic gastroduodenitis. Our results shows that cytoprotective function of the stomach will allow us to develop a differentiated approach in the treatment of chronic gastroduodenitis.
Key words: children, chronic gastroduodenitis, microcirculation of the stomach, collagen IV type, mucopolysaccharides, prostaglandins.
