The experience to prevent renal dysfunctions in term infants with severe perinatal pathology

Purpose — to study the efficacy of a single intravenous dose of theophylline 3 mg/kg of the body weight in the first 60 minutes of life to form renal functions in term infants with clinical signs of severe perinatal pathology. 
 

Patients and methods. A comprehensive clinical-paraclinical examination of 70 term infants at the end of the 1st and 5th day of their lives have been conducted. The main group (ІА) included 25 infants with clinical signs of severe perinatal pathology who in addition to basic therapy received theophylline. The comparison group (ІБ) included 25 infants with analogical condition who received general complex of treatment. The control group (ІІ) included 20 conditionally healthy infants. Renal functions were evaluated on the basis of detection of hourly diuresis, body mass balance, the levels of creatinine and urea in the blood serum, calculation of glomerular filtration rate, and Doppler metric rate characteristics on the major renal arteries. 
 

Results. Term infants with severe perinatal pathology present a risk group to form multisystem lesions including renal dysfunctions. Administration of a single intravenous injection of theophylline in the dose of 3 mg/kg promotes effective stimulation of diuresis and body mass balance, is accompanied by decreased levels of creatinine and urea, increased glomerular filtration rate and positive changes of renal Doppler metric characteristics. 
 

Conclusions. Taking into account positive results of the study conducted, the authors consider it necessary to continue learning the efficacy of theophylline preventive administration for term infants with severe disorders of the early postnatal adaptation processes involving larger cohort of patients with the aim to introduce the presented therapy.
