Features of pregravid training in women with polycystic ovary syndrome due to the insulin resistance in history

Objective: to study the effectiveness of infertility treatment due to the polycystic ovaries in women with insulin resistance. 
 

Patients and methods. The analysis of the effectiveness of the treatment of infertility in women with polycystic ovary syndrome is conducted. The group I consisted of 25 patients in whose diagnosis was done by conventional criteria and determined of the NOMA index as an indicator of insulin resistance. The group II consisted of 25 women with polycystic ovary syndrome with similar methods of examination, but without determination of the NOMA index. 
 

Results. It was found that in all patients whose NOMA index was equal to> 3 and who were treated with metformin were fully resumed ovulation and menstrual function, even without the appointment of additional hormones. In addition, more than half of the women became pregnant. A good treatment outcome had patients whose NOMA index was normal and who were treated in the usual way, because they did not have insulin resistance. Women whose NOMA index was not determined, the effectiveness of the treatment were much lower because the presence or absence of insulin resistance was not proved.
 

Conclusions. NOMA index should be determined in all women with polycystic ovary syndrome, as its level has crucial importance for further tactics of treatment of this disease. Wrong treatment leads to reducing of the likelihood of positive results.
