Diagnosis and treatment of the pharyngonasal cavity streptococcal infection in children in the aspect of prevention of acute reheumatic fever

Objective. To evaluate the diagnostic possibilities and therapeutic tactics of primary care physicians at presence of pharyngonasal cavity infections; outline the ways of management improvement of patients with this pathology for the primary prevention of acute rheumatic fever (ARF).

 

Patients and methods. The 45 medical records of hospitalized patients with ARF who were treated at the Ternopil City Children's Hospital for the last 10 years, at the age of 4–17 years are analyzed. ARF was diagnosed according to the Kisel'-Jones criteria. The conventional clinical and laboratory research tool was conducted for all children. Particular attention is paid to the history data regarding previous streptococcal infection. Statistical analysis was performed using the Microsoft Exel i Statistica — 6.0 program.

 

Results. In 29.0% of children antibiotic treatment of streptococcal pharyngitis in the prehospital stage was not carried out because of the difficulty of his diagnosis etiology, in 16.1% of children the order of the antibiotic therapy was not rational.

Conclusions. The necessity of introducing rapid tests and McIsaac diagnostic scores for the diagnosis and adequate treatment of streptococcal pharyngitis is shown.
