This article is devoted to the actual problem of modern obstetrics – miscarriage. The materials multicenter, randomized, double-blind, placebo-controlled (S.S. Hassan, R. Romero et all., 2011) and prospective placebo-controlled trials (Fonseca et all., 2007) have shown that intravaginal use natural progesterone at risk group positively influences on the process of pregnancy. Syndrome asymptomatic short cervix is one of the manifestations of progesterone deficiency and associated with a high risk of miscarriage and premature birth (level of evidence – A). Timely detection of ultrasound signs of a short cervix and parallel prophylactic intravaginal administration of progesterone allow to prevent preterm birth and perinatal complications / mortality among preterm infants.
