Мета: оцінити рівень NT-proBNP у дітей шкільного віку із вторинними кардіоміопатіями і його зміни при лікуванні препаратом «Агвантар».

 

Пацієнти і методи. Обстежено 53 дитини віком 9–17 років (30 дітей із із вторинними кардіоміопатіями і 23 здорові). Проведено загальноклінічне обстеження, ЕхоКГ, пробу Руф'є, велоергометричну пробу, визначення NT-proBNP у сироватці крові (ІФА).

 

Результати. У дітей із вторинними кардіоміопатіями рівень NT-proВNP у сироватці крові був в межах 51,78±11,02 fmol/ml, що в 4,2 разу перевищувало значення у здорових дітей (12,68±3,05 fmol/ml), і корелював із об'ємними характеристиками серця — КДР (r=+0,52), КСР (r=+0,62), ФВ (r=-0,35), ТЗСЛШ (r=+0,56), ММЛШ (r=+0,51), даними велоергометрії — PWC170 (r=+0,70), МСК170 (r=+0,61), ХР (r=-0,33), ІР (r=-0,46). Застосування препарату «Агвантар» покращило постачання кисню до міокарда і використання резервних можливостей міокарда. Рівень NT-proВNP зменшився удвічі. Тільки у 46,7% дітей в кінці місячного курсу прийому «Агвантару» NT-proВNP перевищував 27,88 fmol/ml.

 

Висновки. Визначення рівня NT-proВNP у дітей із вторинними кардіоміопатіями, поряд із проведенням велоергометричної проби, дозволяє комплексно характеризувати адаптаційні можливості серця і своєчасно виділяти группу ризику щодо формування серцевої недостатності. Застосування препарату L-карнітину «Агвантар» протягом місяця дає можливість покращити толерантність до фізичного навантаження і резервні можливості міокарда, а також нормалізувати вміст NT-proВNP у сироватці крові 55,3% дітей.
