Актуальність. Ко-інфекція ВІЛ/вірусний гепатит В досить поширена у світі, понад 10% ВІЛ-інфікованих осіб мають хронічний гепатит В. Найефективнішим методом профілактики гепатиту В у осіб з ВІЛ-інфекцією є вакцинація. Відповідь на щеплення проти гепатиту В та тривалість поствакцинального імунітету у пацієнтів з ВІЛ-інфекцією можуть бути недостатніми. Метою даного дослідження було вивчення імунного захисту проти гепатиту В у дітей з ВІЛ-інфекцією.


Пацієнти і методи. Дослідження напруженості поствакцинального імунітету проти гепатиту В шляхом визначення рівня анти-Hbs антитіл проведене у 59 ВІЛ-інфікованих дітей, що знаходяться під спостереженням в Київському міському центрі ВІЛ/СНІД та 10 дітей групи контролю, не інфікованих ВІЛ. Всі діти отримали щеплення проти гепатиту В.


Результати. Дослідження показало низький рівень охоплення вакцинацією проти гепатиту В дітей з ВІЛ-інфекцією — 45,7%. Лише 22,0% ВІЛ-інфікованих дітей, щеплених проти гепатиту В, мали захисний рівень анти-Hbs антитіл через 6,3 років після завершення вакцинації. Майже всі діти з ВІЛ-інфекцією отримували антиретровірусну терапію (АРТ). Встановлено, що ранній початок АРТ та відсутність імунодефіциту на час проведення щеплення є предикторами тривалості імунної відповіді після імунізації проти гепатиту В.


Висновок. Ранній початок АРТ і проведення щеплення проти гепатиту В після початку лікування дозволяє зберегти імунний захист проти гепатиту В у дітей з ВІЛ-інфекцією. У дітей, що отримали імунізацію проти гепатиту В до початку АРТ, існує необхідність контролю поствакцинального імунітету і введення бустерних доз вакцини.
