Порушення моторики кишечника при гострих нейроінфекціях у дітей

Гастроінтестинальні ускладнення в пацієнтів із тяжкими формами захворювань асоціюються з підвищенням ризику несприятливих наслідків і негативно впливають на тривалість лікування та строки реабілітації хворих.
Мета — вивчити частоту та клінічні особливості порушень моторики шлунково#кишкового тракту в дітей з гострими нейроінфекціями.
Пацієнти та методи. Дослідження обсерваційне, ретроспективне, типу «випадок—контроль». Проведено аналіз історій хвороби дітей віком від 1 місяця до 18 років, які проходили стаціонарне лікування з приводу гострих нейроінфекцій (менінгіту, енцефаліту та енцефаломієлополінейропатії).
Результати. Дослідження включало 91 дитину з гострими інфекційними ураженнями нервової системи. Симптоми ураження шлунково-кишкового тракту спостерігались у 72,5% дітей. Частота констипації становила 48,4%. Час появи констипації — 80,7±90,0 год. Тривалість епізодів — 90,5±34,2 год. Була виявлена пряма кореляція констипації з тривалістю лікування у відділенні інтенсивної терапії (rpb=+0,48, p=0,026) та проведенням зондового харчування (ВШ=+4,16 ДІ: 1,42–12,00). Прояви діареї відмічались у 34,1% дітей. Термін появи діареї становив 161,5±111,7 год. Тривалість епізодів діареї не перевищувала 3 діб (33,0±19,3 год.). Діарея корелювала з віком дитини (rpb=-0,24, p=0,0002), тяжкістю загального стану (rpb=+0,19, p=0,037), тривалістю перебування у відділенні інтенсивної терапії (rpb=+0,42, p=0,046), клінічною оцінкою тяжкості стану за шкалою РОРС (rpb=+0,19, p=0,037) та рівнем показника лактатдегідрогенази (rpb=+0,67, p=0,0002).
Висновки. Діарея та констипація часто ускладнюють перебіг гострих нейроінфекцій у дітей. Порушення моторики кишечника асоціюються з тяжкістю та тривалістю стаціонарного лікування, віком дитини. Причинами розвитку порушень моторики кишечника є стресова симпатикотонія та цитотоксична дія. Подальші дослідження необхідні для з'ясування механізмів гастроінтестинальних порушень, їх клінічного та прогностичного значення в дітей з гострими нейроінфекціями.
