Особливості хірургічного лікування прогресуючої варикозної хвороби нижніх кінцівок у вагітних

Особливості хірургічного лікування прогресуючої варикозної хвороби нижніх кінцівок у вагітних

Мета дослідження: розроблення комплексу заходів для безпечного хірургічного втручання з метою запобігання прогресування варикозної хвороби нижніх кінцівок у вагітних з мінімальним впливом на перебіг вагітності, розвиток плоду і коректного виношування вагітності.


Матеріали та методи. Хірургічне лікування проведено 76 пацієнткам з прогресуючою варикозною хворобою нижніх кінцівок. Виконано 107 оперативних втручань, переважно у ІІ триместрі (76,3%) і в ІІІ триместрі (23,7%) вагітності у плановому порядку. Використовували для знеболювання у 90,8% вагітних місцеву анестезію: її різновид – тумесцентну анестезію за Дж. Кляйном. Для запобігання розвитку синдрому нижньої порожнистої вени у вагітних під час оперативного втручання вкладали вагітну на операційний стіл на лівий бік під кутом 30°.


Результати. Розроблений комплекс заходів дозволив отримати у 100% вагітних (за їхньою суб’єктивною оцінкою) позитивний результат хірургічного лікування прогресуючої варикозної хвороби нижніх кінцівок й уберегти їх від ускладнень під час вагітності, пологів і у післяпологовий період. Оцінювання результатів хірургічного лікування проводили на 1-й та на 2-й день і через 2 тиж після операції.


Заключення. Тісна співпраця лікаря-акушера і судинного хірурга в спеціалізованому акушерському закладі є визначальною у виборі тактики лікування прогресуючої варикозної хвороби нижніх кінцівок у вагітних. Знання особливостей перебігу вагітності, змін в анатомії і особливості гемодинаміки у венозній системі нижніх кінцівок під час вагітності, виконання малотравматичного хірургічного втручання в оптимальні терміни вагітності висококваліфікованим флебологом дозволяє отримати, за суб’єктивною оцінкою вагітних, 100% позитивний результат.
