Епілепсія у жінок, які планують вагітність

У походженні епілепсії грає роль поєднання двох чинників схильності і органічного ураження мозку. Вивчено безліч публікацій, які стосуються репродуктивних та акушерських проблем у пацієнток з епілепсію. Проаналізована терапія із застосуванням антиепілептичних нападів.

У низці публікацій виявлено, що у більшості пацієнток, особливо тих, які пройшли прегравідарну підготовку, вагітність не погіршує перебіг даної патології.

У публікаціях відсутні переконливі дані на користь вірогідного підвищення частоти розвитку ускладнень вагітності у пацієнток з епілепсією в теперішній час. Але, тим не менш, може відзначатися прямий шкідливий вплив на плід при розвитку генералізованих епілептичних нападів, гіпоксії плода внаслідок розвитку лактат-ацидозу в організмі матері при клінічних судомах, внутрішньоутробна загибель плода при епілептичному статусі.

Пацієнтки з активною епілепсією потребують ефективної терапії під час вагітності. Однак практичному лікарю потрібно пам’ятати про підвищення ризику формування вад розвитку у плода при застосуванні у вагітних АЕП (препаратами вальпроєвої кислоти).
