Білки, що зв’язують жирні кислоти (FABP), як маркери ураження шлунково-кишкового тракту при гострих нейроінфекціях у дітей

Ускладнення з боку шлунково-кишкового тракту (ШКТ) при гострих нейроінфекціях у дітей збільшують ризик несприятливих наслідків і негативно впливають на тривалість лікування та строки реабілітації хворих.
Мета — вивчення частоти ураження органів ШКТ у дітей з гострими нейроінфекціями за допомогою біомаркерів L-FABP та I-FABP.
Пацієнти і методи. Під спостереженням знаходилися діти з гострими нейроінфекціями, що проходили стаціонарне лікування в міській клініці дитячих інфекційних хвороб і яким проводилось дослідження L-FABP та I-FABP у сироватці крові методом імуноферментного аналізу.
Результати. Проведено аналіз 59 історій хвороби дітей із гострими інфекційними ураженнями нервової системи (менінгіт, енцефаліт). При першому дослідженні в день госпіталізації показники I-FABP та L-FABP дорівнювали 8,39±7,89 нг/мл та 228,92±324,19 нг/мл відповідно. При повторному дослідженні (6–7-й день стаціонарного лікування) показник I-FABP становив 7,96±6,67 нг/мл, L-FABP — 166,17±62,04 нг/мл. Найвищий середній показник I-FABP при першому дослідженні відмічався у пацієнтів із нейроінфекціями, що ускладнилися набряком головного мозку (НГМ), а L-FABP — у пацієнтів із септичним шоком. При другому дослідженні спостерігалась тенденція до нормалізації показників I-FABP та L-FABP у дітей із нейроінфекціями, які ускладнені септичним шоком та НГМ.
Висновки. Перебіг гострих нейроінфекцій у дітей часто асоціюється з ураженням ШКТ. Більш суттєві зміни мають зв'язок із розвитком НГМ та септичного шоку. Дослідження біомаркерів I-FABP та L-FABP у дітей із гострими нейроінфекціями дозволяє на ранніх стадіях діагностувати ускладнення з боку органів ШКТ.
