Метаболический синдром: современные взгляды и опасность во время беременности

Приведены современные взгляды на метаболический синдром (МС) и его отрицательное влияние во время беременности. МС, по утверждениям экспертов Всемирной организации охраны здоровья, является «пандемией XXI века» в связи с его распространенностью среди населения, высоким риском развития сердечно-сосудистых заболеваний и преждевременной смертности. В рутинной клинической практике диагноз МС устанавливают при наличии хотя бы трех факторов риска: абдоминальный тип ожирения; высокий показатель триглицеридов; низкий показатель холестерина липопротеидов высокой плотности; повышенное артериальное давление; повышенный показатель глюкозы натощак. К проявлению МС относят склонность к тромбообразованию, печеночный стеатоз/стеатогепатит, микроальбуминурию/нефропатию, энцефалопатию, нейросенсорную тугоухость, желчно-каменную болезнь, поликистоз яичников, гиперурикемию, синдром обструктивного апноэ во время сна, остеопороз, патологию периодонта, импотенцию/сексуальную дисфункцию и др. Влияние негативных факторов в детском и подростковом возрасте (неправильное питание, низкая физическая активность, курение) приводят к развитию МС. МС во время беременности обуславливает метаболические нарушения в системе гемостаза. Физиологически во время беременности повышается риск тромбоэмболических осложнений (по некоторым данным, в 4–5 раз) по сравнению с состоянием до беременности. МС ассоциируется с гиперкоагуляцией за счет повышения активности плазматического звена гемостаза, снижения фибринолиза, появления эндотелиальной дисфункции, повышения активности тромбоцитов. Эти нарушения в системе коагуляции и фибринолиза могут быть дополнительными факторами, ухудшающими течение беременности и повышают риск тромботических осложнений у пациенток с МС.
