Эффективность диагностики и лечения преинвазивного и микроинвазивного сквамозоклеточного рака шейки матки

Цель исследования: анализ эффективности диагностики и лечения больных с ранним раком шейки матки.
Материалы и методы. Проанализированы данные 107 женщин, больных СIN ІІІ, сancer in situ, которые находились на лечении в Киевском областном онкологическом диспансере и Национальном институте рака МЗ Украины с 2010 по 2015 год, в возрасте от 20 до 52 лет, – истории болезни, амбулаторные карточки, гистологические препараты.
Результаты. Процент дагностированных рецидивов СIN ІІІ, сancer in situ составил 4,57%, неинвазивных и инвазивных форм – 0,94%.
Заключение. При выявлении СIN ІІ і СIN ІІІ рекомендовано проводить лечение путем диатермоконизации и динамическую диспансеризацию, применяя комплексную программу (кольпоскопия, цитология, онкомаркер SСС, выявление инфицирования HPV 16,18).
При наличии в крае резекции элементов опухоли после расширенной диатермоконизации, при формах сancer in situ, множественных экспансивных очагах в железах шейки матки, реализованной репродуктивной функции, сопутствующих заболеваниях (узловая лейомиома тела матки и т.д.) следует рекомендовать гистероскопическую радикальную трахелэктомию.
Перспективным является принятие «принципа продолжения наблюдения» и скрининговое выявление инфицирования HPV 16,18 при СIN І, CIN ІІ.
