Refining definition of «palliative care»: importance and impact on the development of the palliative care system in Ukraine

Objective: the medical and social analysis of the definition of «palliative care» and its importance and impact on the affordable, highquality and effective palliative care (PC) system in Ukraine in the context of the health care system reforming.
The national and international normative legal documents, national and foreign scientific literature, medical statistics were reviewed, and the following research methods were applied: comparative content analysis, information analysis, statistical method, system approach.
A comparative analysis of the normative legal documents of Ukraine and authoritative international organizations shows that the definition of «palliative care» in the Ukrainian legislation is somewhat inconsistent with modern international practice. Whereas to date, in accordance with the legal and regulatory framework of Ukraine, the palliative care is a kind of medical care and concerns only the terminally ill patients with predicted (limited) life expectancy and their family members, while WHO defines palliative care as an approach that improves the quality of life of patients and their families facing the problem associated with life-threatening illness. Of course, such an approach greatly extends the segment of the population and increases the need for PC. Medico-statistical data testify to the rapid population aging in Ukraine and in the world, the increasing of morbidity associated with life-threatening and incurable diseases, in particular cancer and chronic non-infectious and infectious diseases, and the low availability of palliative care for the population. The multidisciplinary and intersectoral model of the palliative care in the inpatient and outpatient settings, including home care, the conditions for the creation and implementation of affordable, high-quality and effective palliative care system in Ukraine are substantiated in the article.
The authors demonstrate the importance and significance of the PC strategy development in Ukraine, based on internationally recognized definition, and discuss possible models and integration levels of PC in public health care.
