Operating results perinatal centers in Ukraine.

By the beginning of 2014 in Ukraine created and operates 12 perinatal centers level III (PC III level), it was preceded by considerable work on the preparation and implementation of the regulatory framework, training, introduction of new health technologies and organizational security regionalization of care for pregnant women, women and childbirth. The article summarizes the first results of the monitoring activities of the HRC in the country who have shown the effectiveness of the technologies used. So, in 2013, the HRC Level III was born 57% of pregnant women with congenital heart disease, 34,3% with diabetes, 33,3% with severe pre-eclampsia and eclampsia in Ukraine as a whole. During the year 59% of infants born weighing 500,0-999,0 g and 50.5% – weighing 1000,0-1499 g, 27% of infants with congenital malformations; twice dropped from 6% to 2,9% share of newborns who are transferred to children's hospitals for follow-up care, while the share of reasonably fruit and maternal deaths that occur at HRC III level decreases such I and II levels of care.

Quality of life and the role of menopausal hormone therapy.

The quality of life of countless menopausal women world-wide has been signifi cantly diminished following the sensationalist reporting of the Women’s Health Initiative (WHI) and the resulting 50% or more decline in the use of hormone replacement therapy (HRT) over the subsequent 10 years. Quality of life is diffi cult to measure as there are so many contributing factors and a large number of different instruments, some of which assess general health and only a few which specifi cally include symptoms related to menopause. HRT improves quality of life of symptomatic menopausal women and some studies of the effects of HRT provide reliable evidence on quality of life other than reduction in vasomotor symptoms. Until there is a better understanding of the minimal risks of HRT for the majority of women, too many will continue to suffer a reduced quality of life unnecessarily.

Alternative approaches to follow-up a missed abortion: controversions of surgical and conservative methods.

Analysis of the results concerning publications dedicated to a missed abortion (MA) is given. Some advantages of the alternative method usage – a medicament abortion in early pregnancy terms in the first trimester – over a traditional surgical treatment are discussed in the article. A mechanical enlargement of cervical duct and scraping of uterine cavity walls are the standards of providing medical aid in case of MA. Additional confirmation of structurally and functionally defective endometrium, risk of potential complications of surgical removal of a non-developing fetal ovum as well as risk associated with anaesthesia create a treat to reproductive health of a woman. Therefore, issues concerning merciful evacuation of uterine cavity in case of MA are extremely topical. Wide popularization of the method requires performance of additional multicentered studies in this direction.

Experience with abdominal sacrocolpopexy as a prevention method of posthysterectomy vaginal vault prolapse.

A new method of abdominal sacrocolpopexy was proposed at patients with high risk of posthysterectomy vaginal vault prolapse. High fixing of a vaginal cupule to sacro-uterine ligaments with reconstruction of fascia is a preventive measure of posthysterectomy vaginal vault prolapse.

First experience of new domestic combined colloid and hypertonic solutions in intensive therapy of hemorrhagic shock.

The article presents study of domestic combined solution Hekoton conducted at the departments of anesthesiology and intensive care of the Kiev Center of Maternal and Child Health and Kyiv Regional Clinical Hospital. Our experience in the use of this preparation were 21 patients with acute blood loss, traumatic shock. We studied the blood pressure, the heart rate, urine output, the state of the microcirculation. The results of our study revealed efficacy of Hekotonu. The advantage of this preparation also include balanced pharmaco-economic component. This allows us to recommend it to widespread use as a «starter» drug in treatment of hypovolemia and shock of varying degrees, which allows in short time achieve the desired therapeutic effect and reduce the risk of complications.

Vaginal birth in women with a uterine scarclinical experience.

During the study we have identified the selection criteria, the main indications and contraindications for vaginal birth in women with a uterine scar. According to the obtained results, it follows that with proper identification and compliance of safety criteria about 20% of pregnant women with a uterine scar can successfully deliver.

The echoqraphic features of existing tubal pregnancy.

The goal of investigation is determination of echographic indicators of uterus endometrium of pregnant with developing tubal pregnancy. There was examinet 34 woman with wxisting tubal pregnancy. The clinical examination included ultrasound investigation of leng, width, antero-posterior diametr of the uterus, thеakness of endometrium, sires of ovum with vaginal transduser. It was revealed that there is no difference between the echographic indicators of uterine endometrium at developing tubal pregnancy and normal uterus without pregnancy. The probability of ovum visualization increases at delayed menstruation more than 14 days. The visualization of embryo with yolk sac inside in the tubal area is positive at the late periods of missed menstruation.

Cytokine balance peculiarities in pregnant women operated for the thyroid gland during pregnancy.

The peculiarities of cytokine metabolism in women operated on the thyroid gland during pregnancy before and after surgery are represented in this publication. It was stated that the cytokine dysbalance is more meaningful in pregnant with diffuse toxic goiter. The surgery with the consequent hormone replacement therapy had the positive influence on the thyroid hormones metabolism. The hormone replacement therapy by micronized progesterone in women with signs of threatened abortion led to the improvement of pregnancy cytokine paradigm.

The features of fetoplasentar blood circulation of pregnant with inflamatory desease of kidneys and urinary tract.

The goal of our investigation is estimating of condition of utero-placental and fetoplacental blood suply of pregnant with inflamatory deseases of kidneys and urinary tract. Using dopplerography were examined the blood circulation in right and left uterine arteries, umblical artery, middle cerebral artery of the fetus, vessels resistancy index, pulsatily index and average speed of blood circulation in arteries. It was achieved that the early signs of fetoplacental insufficiency increasing of resistancy index and decreasing of average speed of blood circulation in uterine and umblical arterias.

Effect of pregravid preparation on the course vaginal birth among women with uterine scar.

This article describes the perspective of reducing the frequency of repeated cesarean section on account of vaginal birth among women with one scar on a uterus with the help of pregravid preparation. To make a complex of pregravid preparation that includes immunostimulating, absorbable and vitamin therapy in combine with hormonal correction, antioxidants prescription as a preventive actions, psycho prophylaxis, diagnostic hysteroscopy, the course of physiotherapeutic procedures – amplipulse therapy, stimulates the reducing of the pregnancy complications, increases the probability of the successful vaginal birth attempt.

Comparative analysis of the uterine involution of primiparous and multiparous women within 48 hours after labor.

Uterine involution is the most appropriate predictor of current postpartum period. The article presents results of the comparative analysis of uterine contractile activity during the first two days after physiological labor of primiparous and multiparous women. It’s determined that physiological course of postpartum period is characterized by simultaneous decrease of almost all studied sizes of the uterus during the first 48 hours after labor. The only exception is the length of the cervix of the uterus, that increases gradually. The biggest changes occur within first 12 hours after labor, that defines exactly this term for screening of subinvolution and pathology of involutive function of the uterus, both for primiparous and multiparous.

Peculiarities postoperative period in women depending on the method abdominal delivery.

This paper presents data on the characteristics of the course of the postoperative period, depending on the method of abdominal rodorozrishennya, urhentnosti category, presence of comorbidity. 1. For hemorrhage: the least blood loss during elective surgery in Derfleru method and by the method of Stark. 2. For the duration of the operation: the method of Stark regardless of category urhentnosti lowest duration of surgery (41–44 min). 3. For the duration of maternity hospital in the postoperative period: lowest length of stay during routine operations, regardless of the method of cesarean section (4,1±0,03 days). 4. For the percentage of complications in the mother had no complications during the operation of Derfleru, but the result is little information as held 26 (8,7%) operations given by the total number of transactions. 5. In the state of the newborn: during the operation routinely regardless of the method of cesarean section one of the children is not transferred to the neonatal intensive care unit. In conducting clinical and laboratory examination in women three months after operative delivery anemia often manifested in women who reoperated (35,8%) in urgent procedure (26,3%), the method Derflera (46,1%). Pathological cytological picture usually accompanied by clinical signs of inflammation of the vagina. The second degree of purity of vaginal contents were diagnosed in 94 women (31,4%), and the third – in 34 women (11,3%). These are mainly patients who are operated on again in urgent order, regardless of the method of cesarean section.

Evaluation of the efficiency of rehabilitation measures using pharmatex (benzalkonium chloride) in women with diseases of the cervix uteri after destructive treatments.

Sixty-five radiowave-treated patients aged 20 to 32 years with diseases of the cervix uteri were examined. In the postoperative period, 34 patients from the study group used Pharmatex vaginal cream once daily for 20 days; 31 women (a comparison group) received a combined (dexpanthenol 0.1 g and chlorhexidine bigluconate 0.016 g) agent intravaginally for 10 days. Clinical, laboratory, instrumental, and morphological studies were conducted in all the women. There was evidence for the well tolerability of benzalkonium chloride (Pharmatex cream), accelerated epithelization processes in the cervix uteri, and a significant reduction of complications (lymphorrhea, hemorrhage) in the postoperative period, which may recommend Pharmatex as a cream for wide clinical application.

The features of cytokine status of women with early pregnancy loss.

The cytokine status is studied in women with threatened abortion and miscarriage in terms of 4–11 weeks of pregnancy. Increase of production of proinflammatory cytokines IL-1β, IL-6 and decrease of concentrations of anti;inflammatory cytokine IL-4, IL-10 indicating inappropriate fetoprotektiv trigger mechanisms. Studies of cytokine status is important for the evaluation of the immune response, forecasting the development of gestational complications and pregnancy outcomes for mother and fetus: changes in the balance of pro- and anti-inflammatory mediators at the local and systemic level are the criteria of threatened abortion.

Biocenosis and functional activity of an epithelium of a vagina at local treatment of an aerobic vaginitis polygynax and tergynan. Multicenter randomised controllable comparative research.

The given research is devoted comparative studying of efficiency and safety of preparations polygynax and tergynan in treatment of an aerobic vaginitis (АV). Estimated clinical (degree of reduction of objective and subjective symptoms) and microbiological efficiency of therapy by the specified medical products at the patients suffering АV. Besides, studied rate of disappearance of an inflammation of a vagina, functional activity of an epithelium of a vagina, influence of preparations on a condition of a normal microflora of a vagina upon termination of therapy and spent an estimation of quality of a life before therapy АV. For the decision of tasks in view investigation and treatment of 60 women of genesial age (18–45 years) with diagnosis АV is carried. Research is executed according to the international specifications and rules GCP. As a result of the carried out research it has been established that polygynax and tergynan are comparably effective and safe at treatment АV. Polygynax and tergynan are equally effective the relation of is conditional-pathogenic microorganisms – originators AV. Polygynax is a preparation of a choice for treatment АV. Tergynan it can be applied as a complex preparation to treatment bacterial vaginosis and АV. Polygynax does not render immunosuppressive action on a vagina epithelium thanks to what restoration of functional activity of an epithelium after treatment polygynax occurs faster, than after treatment tergynan. After treatment quality of a life of patients as a whole has improved in each of studied groups.

In this article we can find the data on the modern ways of treating patients with adenomyosis . A series of publications and our own studies show that a rational pathogenetic treatment in patients with inner endometriosis enable to increase clinical effectiveness of the treatment mentioned above. This treatment is, first of all, directed to the realization of organ saving tactics and reconstruction of reproductive function in women patients. However, hormonal methods of therapy are not the only way of treating patients. Working on new schemes and methods of therapy can increase the positive chances of patients’ treatment with inactive adenomyosis, and can stop it’s turning into active form.

Analysis effectiveness of the methods of infertility treatment amongst females with PCOS and hyperprolactinemia: retrospective study.

The study was aimed to assess the clinical effect of the various methods of infertility treatment amongst females with PCOS and hyperprolactinemia. There was demonstrated that the multifactorial infertility i.e. combination of hyperprolactinemia and PCOS occurs amongst every third female with fertility disorders. The differentiated use of the conservative medication treatment allows restore fertile function in 33.8%. There was stated that the surgical induction of ovulation by the laparoscopic partial ovarian destruction allows increasing the effectiveness of the complex treatment of infertility up to 40%. There is discussed the expedience of the development of the algorithm of the differentiated use of the methods of infertility treatment amongst females with PCOS and hyperprolactinemia.

The use of combination of ciprofloxacin and tinidazole for prevention of purulent-inflammatory complications after minor gynecologic surgery.

Despite permanent improvement of technical and pharmacological aspects of ensuring the efficiency and safety of minor surgical interventions in case of gynecological pathology, the frequency of purulent-inflammatory complications can substantially increase. There is a wide range of antimicrobial drugs for prevention and treatment of purulent – inflammatory diseases in women in the postoperative period after minor gynecologic operations. Antimicrobial agent should have antianaerobe efficiency, cause rapid bactericidal action, have high antimicrobial activity, be stable to β-lactamase, easy to use and dosage, have minimal toxicity and maximum safety. Such requirements for minor gynecologic surgeries correspond to combine of ciprofloxacin and tinidazole. The range of action of preparation covers gram-negative bacteria: Escherichia coli, Klebsiella, Salmonella, Рroteus, Shigella, Enterobacter cloacae, Morganella morganii, Providencia, Citrobacter, Serratia marcescens, Campylobacter jejuni, Pseudomonas aeruginosa, Neisseria gonorrhoeae, Haemophilus influenzae, Haemophilus parainfluenzae, Bacillus anthracis., Moraxella catarrhalis. Сombination of ciprofloxacin and tinidazole is effective against gram-positive bacteria: Staphylococcus, Streptococcus, including Streptococcus рneumoniae, Listeria monocytogenes, Corynebacterium spp. Tinidazole causes antiprotozoal action against Trichomonas vaginalis, Entamoeba histolytica, Giardia lamblia; bactericidal activity against anaerobic bacteria – Bacteroides spp., including Bacteroides fragilis, Bacteroides melaninogenicus, Clostridium spp., Eubacterium spp., Fusobacterium spp., Peptococcus spp., Peptostreptococcus spp., Veillonella spp. The advantages of combination of ciprofloxacin and tinidazole use after minor gynecologic surgical interventions for prevention of purulent-inflammatory complications are its high effectiveness against all major pathogens, maximum safety and tolerability, the high stable concentration in the possible areas of infection and economic feasibility.

Results of the spent researches testify that patients with reproductive losses in anamnesis differed from women of group of comparison under following characteristics: to number of sorts, frequency of smoking, an index of somatic health, age of a sexual debut and a condition of reproductive health. Problematic aspects of cervical pathology in women with pelvic pain syndrome of adenomyosis.

The article is devoted to one of the most topical problems of gynecology – the problem of pelvic pain caused by adenomyosis and cervical pathology. The specific frequency of adenomyosis in the structure of genital endometriosis reaches 70–90%. The interest to the problem lies in the fact that the simultaneous synchronous involvement in the pathologic process muscle and an endometrial tissue and connective tissue components extracellulyar matrix, can’t touch the epithelial tissue of the cervix. The study involved 24 women of reproductive age with pain syndrome of adenomyosis as a core group and a comparison group consisted of 10 women of similar age without painful disease. The results of the study showed that in patients with pain syndrome of adenomyosis in the reproductive age are quite common related pathological conditions in the cervical epithelium with the prevalence of the virus associated intraepithelial lesions.

The results of study of microbial spectrum of urogenital organs among women with changes in reproductive health which accompanied by violations of autonomic homeostasis are showed in the article. Obtained data shows changes in microecology of the genital tract and their connection with autonomic homeostasis disorders among fertile aged women with violations of reproductive health, and need in further control and observation.

Optimization of the treatment of chronic hyperandrogenic anovulation in patients with endocrine infertility on the background of PCOS.

The article presents the results of a clinical study of therapeutic efficacy and safety of the drug containing flutamide, which was attended by 86 patients with polycystic ovary syndrome (PCOS) with menstrual irregularities and hirsutism. Investigations were carried out in two stages: clinical and laboratory examination (during the menstrual cycle) and treatment (3–6–9 months). Patients who met the inclusion criteria, received anti;androgenic drug flutamide containing a daily dose of 125 mg three times a day for 6 months. The study confirmed that the use of flutamide for the treatment polycystic ovary syndrome that cycle is accompanied by disturbances and hirsutism, contributes to more efficient recovery of reproductive potential in women.

Based on studies conducted have determined that treatment with natural origin Tribestan. patients with chronic pelvic inflammatory disease contributes to limit the manifestation of exudative inflammation, inactivation of inflammatory mediators, and the restoration of local iimuniteta obtaining 95.3% of the positive therapeutic effect, which makes extensive use of this drug in the treatment of chronic forms of PID.

Impact assessment of electric welding biological tissues at a rate of postoperative complications in patients with benign tumors of the brest.

Postoperative complications were studied in 156 patients with benign tumors of the breast. 76 patients was performed traditional sectoral resection, 80 patients conducted surgery using electric welding machine EK- 300M1. A comparative analysis of postoperative complications set significant advantages of using electric welding of biological tissues over the traditional method of operation. Using electric welding biological tissue during the operation such complications as suppuration of wound, marginal necrosis, surgical erysipelas inflammation significantly decreased by 7,7–10,6%. Bleeding during surgery and postoperative hematoma not met even once. Operative time decreased by an average of 6,2±2,3 minutes.

Results of the spent researches testify that the type of placentation and especially presence abnormal placentation influences a functional condition fetoplacental complex, and also development of perinatal pathologies. For timely diagnostics placentary dysfunction and frequency decrease perinatal it is necessary for a pathology early ehografical inspection of localisation of a placenta with the subsequent dynamic inspection of patients for an estimation of processes of migration of a placenta. The way and terms delivery women with abnormal placentation depend on localisation of a placenta, completeness of its migration and a functional condition of fetoplacental complex.

Screening of pregnant women is a complex system that includes not only conducting certain tests and examinations, but also provide a pregnant family with adequate information about the disease, which is aimed at finding the present screening test, and in case of fetus disease detection – about modern possibilities of treatment or abortion. The introduction into clinical practice of modern automated approach of calculating risk in non-invasive prenatal prevention allows to optimize and dramatically increase the effectiveness of screening. The article highlights contemporary approaches to prenatal diagnosis of chromosomal abnormalities and systemized the main key points that determine the conduct of high-quality prenatal diagnosis.

The modern aspects of combination therapy of cervical neoplasia associated with HPV infection.

Human papillomavirus (HPV) have been identified in the most cases of cervical precancerous disease and cervical carcinomas. Several factors may be associated with higher rates of surgical treatment failure, including highgrade CIN, large lesion size, satellite HPV-related lesions, involved margin, persistent of HPV infection. The purpose of our study was to evaluate the effect and toxicity of the antivirus therapy of cycloferon in the complex treatment of HPV-positive women affected CIN I and CIN II after radio-wave excision. According to analysis of our investigation we can conclude that cycloferon is safe and necessary component of the treatment of precancerous cervical pathology. Surgery with the antivirus therapy of cycloferon promotes elongation interrecurrent period, cessation of viral shedding, accelerates reparative processes in the areas of damage, saves of the architecture of the cervix and vaginal biocenosis normalizes.

The mechanisms of immune defence are examined that take place in the organism of man at infikuvannii by the onkogennimi cultures of papillomavirus infection. The presented information is about specificity of mechanisms of slipping out of tumours from the immune system. Scientific basis of immunosupresivnikh properties of onkobilkov of papillomavirus infection is exposed.
