Prevention of infections in patients with asplenia or dysfunction of the spleen

An impairment of all spleen functions is observed in congenital asplenia as well as in asplenia caused by an injury to the spleen. There also exists a group of diseases that lead to functional functional hyposplenia. Dysfunction or absence of the spleen increases the risk of life-threatening infections. Methods of preventing the disease in patients with congenital, acquired or functional asplenia include three main areas: vaccination, chemoprophylaxis and educating patients, parents and medical staff. This article outlines recommendations for the prevention of life'threatening infections in this group of patients. Patients should undergo vaccination against Streptococcus pneumoniae, Haemophilus influenzae type b, Neisseria meningitidis, and annually against influenza. Oral penicillin antibiotic or macrolides prophylaxis is conducted, but recommendations for its duration vary in different countries (from one year to lifelong in high risk groups). In Ukraine there are no unified guidelines how to provide medical care to such patients, that is the reason of urgent demand for updated protocols approach for all medical specialities.
