Дифференциальная диагностика синдрома запора у детей

Запоры наблюдаются у 3% дошкольников и у 1–2% школьников. У большинства детей, которые обращаются за медицинской помощью по поводу запоров, не обнаруживается органического заболевания.

Цель: повышение эффективности проведения дифференциальной диагностики синдрома хронического запора у детей.

Нарушение двигательной активности толстой кишки является ведущим механизмом образования данного синдрома. Большое значение в патогенезе хронического запора имеет нарушение структуры (воспалительные, склеротические, опухолевые процессы) и функции прямой кишки (координации деятельности мышц тазового дна со сфинктерным аппаратом прямой кишки). Основные клинические проявления запоров: частота стула два и менее раз в неделю; болезненная дефекация; отказ от посещения туалета; твердый кал; каломазание; абдоминальный дискомфорт. Для постановки предварительного диагноза достаточно данных анамнеза и физикального обследования, однако для окончательного вывода необходимо проведение тщательного лабораторно/инструментального исследования для исключения вторичной природы запора, особенно органических причин его происхождения. Разное происхождение запоров, сложный механизм их развития, необходимость дополнительных методов диагностики и назначения в каждом конкретном случае индивидуального лечения определяет важность дифференцированного подхода к данной проблеме.
