Базовые аспекты педиатрической паллиативной помощи

Рассмотрены основные составляющие педиатрической паллиативной помощи (ППП) и особенности ее оказания. Представлены 11 категорий состояний, которые обуславливают потребности в паллиативной помощи. Установлено, что наибольший вклад принадлежит врожденным аномалиям, перинатальным состояниям, белково-энергетической недостаточности, менингитам и ВИЧ/СПИДу. Педиатрическая паллиативная группа отличается высокой гетерогенностью, обусловленной широким возрастным диапазоном; широким спектром нозологий, в том числе орфанными заболеваниями; разницей в потребностях в зависимости от возраста и развития в среде амбулаторных, лечебных учреждений и дома. Определение ППП и ее цель представлены соответственно терминологии ВОЗ, ведущих паллиативных фондов и Триестской хартии прав умирающего ребенка. Акцентировано внимание, что отличия педиатрических паллиативных категорий состояний базируются на траекториях смерти ребенка и обуславливают потребности в паллиативных сервисах. Обсуждены модели ППП и уровни ее оказания, особенности работы детских хосписов. Полная система ППП представлена интегрированной трехуровневой помощью: паллиативный подход, общая паллиативная помощь, специализированная паллиативная помощь. Детально рассмотрены особенности ППП, связанные с ребенком и его семьей, готовностью сети педиатрических учреждений и подготовкой врачей по вопросам оказания паллиативной помощи детям. Особенностью ППП является необходимость обеспечения сервисами с момента установления диагноза неизлечимой болезни, несмотря на прогнозированную длительность жизни. Такая помощь должна быть оказана в любом месте, независимо от модели лечебного учреждения, и так долго, насколько это необходимо, а начало паллиативного ухода не означает помощь только в конце жизни и отказ от борьбы. Рассмотрены основные принципы управления базовыми симптомами, особенно обращено внимание на важность оценки боли, уровень боли и базовые принципы контроля. Общий холистический подход при организации ППП позволяет использовать все возможности для поддержания сил ребенка с ограничивающим жизнь заболеванием.
