Вагинальные таблетки (Гинофлор®), содержащие очень низкую дозу эстриола и Lactobacillus acidophilus, при вагинальной атрофии у пациенток в постменопаузе, которые страдают раком молочной железы и применяют ингибиторы ароматазы клиническое исследование фармакокинетики, безопасности и эффективности (фаза I)

Было проведено фармакокинетическое (ФК) исследование (фаза I) для оценки уровня циркулирующих эстрогенов у пациенток с раком молочной железы, которые применяют нестероидный ингибитор ароматазы (НИА) при вагинальной атрофии и используют комбинированные вагинальные таблетки, содержащие очень низкую дозу эстриола (Е3) – 0, 03 мг и Lactobacillus (Гинофлор®). Шестнадцать пациенток, применявших НИА и у которых диагностирована тяжелая вагинальная атрофия, ежедневно применяли по 1 вагинальной таблетке препарата Гинофлор® течение 28 дней, после чего наступал этап поддерживающей терапии с использованием 3 таблеток в неделю в течение 8 нед. Основные показатели включали сывороточную концентрацию и фармакокинетику эстриола (Е3), эстрадиола (Е2) и эстрона (Е1), определенную методом чувствительной газовой хроматографии ; масс-спектрометрии. Дополнительные показатели включали клинические оценки эффективности и побочные эффекты; микроскопические изменения вагинального эпителия и микрофлоры, а также изменения сывороточного уровня ФСГ, ЛГ и глобулин-связывающего полового гормона. Сывороточный уровень Е1 и Е2 по сравнению с исходным уровнем не повысился ни у одной из пациенток после применения вагинального средства. Сывороточный уровень Е3 временно повышался после первого применения у 15 из 16 пациенток, максимальный уровень составлял 168 пг/мл через 2–3 ч после введения препарата. Через 4 нед. сывороточный уровень Е3 был незначительно повышен у 8 пациенток, максимальное значение составляло 44 пг/мл. Вагинальная атрофия была ликвидирована или ее симптомы улучшились у всех пациенток. Пациентки хорошо переносили препарат, никто не прекратил терапию. Применение вагинальных таблеток, содержащих низкую дозу Е3, 0,03 мг и Lactobacillus acidophilus, у пациенток в постменопаузе с раком грудной железы при применении ингибитора ароматазы, страдающих вагинальной атрофией, привело к незначительному и временному повышению уровня Е3, но не уровня Е1 или Е2, поэтому этот препарат можно считать безопасным и эффективным средством для лечения атрофического вагинита у пациенток с раком молочной железы, которые применяют НИА.
